Krehling was issued two violations in 2001 for a similar incident involving catching and crushing in August 2000. A crane struck and pinned 21-year-old Ramon Meza Barrera against a wall, killing him. The company was cited for 23 serious safety violations, which included lack of railings, defective stairs and no hard hats. In total, the violations cost the company $94,500 in fines.

The Department of Labor's Occupational Health and Safety Administration cited and imposed a potential $1,375 fine against JDM Materials Co. Inc. of Philadelphia and Huntingdon Valley, the company that hired the three contractors who died.

According to a state report released Monday, Klee Construction Co. committed eight code violations – including one willful violation – in allowing its employees to work at a site with obvious potential for disaster.
- 1/07/04: Jackson, Mississippi OSHA has issued 11 serious citations to LeTourneau, Inc., for exposing workers to hazards following the investigation of a fatal accident that occurred July 28 at the company's Vicksburg facility. The citations carry proposed penalties totaling $42,500.

The federal Occupational Safety and Health Administration fined Conewago Enterprises $7,000 -- the maximum the agency can assess -- over the training of the employee who was operating a crane involved in the Jan. 16 death of William D. Coulter, 44, of Fairfield, Adams County.

TRENTON, N.S. (CP) -- A Nova Scotia rail-car maker has been fined after an employee was crushed in a workplace accident earlier this year.

Investigations showed that Chong Kee Ling was operating the crane in an unsafe manner and had overloaded it when the accident occurred in February this year. The incident caused the death of a worker.

A Florida-based construction firm was cited today for the second time this year by the
Occupational Safety and Health Administration for knowingly exposing workers to
potentially fatal fall hazards at a bridge near Elizabeth, N.J. OSHA is proposing penalties
totaling $217,500.

OSHA Fines Texas Steel Company More Than $1.7 Million for 180 alleged violations of workplace safety and health standards. OSHA cited U.S. Denro Steel Inc., doing business as Jindal United Steel Corporation (JUSC), with a total of 182 alleged violations, 126 of which are categorized as willful.

This is the second electrocution of a Kohler Construction Company employee at a Florida construction site within the past three years.

"We are deeply troubled by the history of fatal accidents at this company," said Labor Secretary Alexis M. Herman. "Effective safety programs can and must prevent tragedies like these. The department expects companies to take the necessary steps to protect workers in dangerous jobs. We will take appropriate enforcement action to see that they do."
Since 1972, 37 workers have been killed on L.E. Myers jobs, including 15 who were electrocuted. L.E. Myers has 1,500 employees nationwide; no other company its size in its industry has as many fatalities.

An Edmonton sandblasting company where a city man was crushed to death was handed a hefty fine yesterday after pleading guilty to safety violations.
However the $45,000 penalty - said to be the second-highest fine handed out in Alberta - isn't much compared to the value of the man who was killed, said his ex-wife.

Vanguard Manufacturing, Inc.
The U.S. Labor Department's Occupational Safety and Health Administration (OSHA) has
cited Vanguard Manufacturing, Inc., a New Ipswich, New Hampshire, maker of metal
scaffolding, for alleged Willful, Serious and Other than Serious violations of the
Occupational Safety and Health Act at its Temple Road plant and has proposed penalties
against the company totaling $51,000.
“Cranes and slings which had not received the required frequent and periodic inspections and a defective sling had not been removed from service.”
